Early identification of patients requiring palliative care
Identifying patients who would benefit from a palliative approach earlier in their disease trajectory is essential to ensuring patients have access to the services they need, at the right time, at the right place, by the right care provider.
The South West Hospice Palliative Care Network in partnership with the South West Regional Cancer Program has launched INTEGRATE: early identification of patients requiring palliative care - a regional palliative care improvement project. The goal of the INTEGRATE project is to drive system improvement through the earlier identification and coordination of palliative care services in London Middlesex (and across the South West Region), providing a seamless, integrated system of care for patients.
